ACMC's patient-centered Medical Home brings patients and their care providers together as a team to improve each patient's health and quality of life.

What is Medical Home?

A Medical Home is a partnership between the patient, the primary care physician and the healthcare team, including a care coordinator, who work together to provide the best total healthcare.

Medical Home isn't just about having someone who can give you medical care when you need it; it's also a focus on prevention and improving your overall quality of life so you can stay healthy and independent. Your care is tailored based on your health and family history. Using your health information, you and your care team work together to make the best decisions about your health.

No matter how often you see your doctor or how many doctors are involved in your care, Medical Home may be for you. Each patient's Medical Home will differ depending on their medical needs, but every Medical Home is led by a primary care physician and a patient care coordinator. Medical Homes typically benefit those patients with diseases that persist for long period of times or those who see a number of doctors.

Medical Home puts an emphasis on patient-centered care: What are the patient's concerns? What are their goals? How can we meet these goals from a healthcare standpoint, while identifying gaps in their healthcare to improve the quality of life for each individual?

ACMC's Medical Home gives patients the opportunity to build stronger relationships with their individual providers and get the benefits of a healthcare team working together to make every patient as healthy as possible. Each patient participating in Medical Home has a care coordinator, who is available to answer calls and questions, remind them about check-ups and tests and help coordinate their care with specialists. Medical Home provides the tools needed to lead a healthier life.

ACMC encourages each patient to be informed and involved in their healthcare decisions. Patients work with their care team to develop personal goals to ensure optimum health goals, including how to reach these goals outside of your visits with your primary care provider.

Making the Most of Medical Home

With ACMC's Medical Home, patients are encouraged to speak up about their health. There are a number of ways you the patient can make the most of your Medical Home:

  1. Speak up about your health.
    1. Speak up about concerns or questions you have. If you still don't understand ask again.
    2. Know your medications. Know what medications you take and know why you take them.
    3. Participate in all decisions about your treatment and the healthcare services you receive.
    4. Tell your team how you feel about the care you are receiving from them.
  2. Take care of your health.
    1. Follow the personal care plan you and your team will develop together.
    2. Set goals you can reach.
  3. Talk openly with your team.
    1. If you feel your care plan is not working or you're having troubles, speak up. Tell your team what is not working so together you can make changes if needed.
