This website permits you to access a "Follow My Health" patient portal through a link on the ACMC website. Clicking on the link for "MyHealth@ACMC Patient Portal" under the Patient Resources tab will take you to a page through which you may review, enter, and modify your health information and related personal and demographic information after you have set up your Follow My Health account.
Our online communication practices
General e-mail communications
Most e-mail, including the e-mail functionality on our site, does not provide a completely secure and confidential means of communication. It is possible that your e-mail communication may be accessed or viewed inappropriately by another Internet user while in transit to us.
